Steps to ensure you submit accurate information:

• Provide the employee's name exactly as listed on their SSN card.
• Enter double or hyphenated last names together in the last name field.
• Only use a hyphen only if shown on the SSN card.
• Only include the middle name if it is listed on the SSN card.
• If there is no middle name, leave the middle name field blank.
• Double check your report for possible typos and transposed numbers.
• Be sure all written or typed lists are clear and easy to read.
